Once you're past the basics, a few dosing questions tend to come up. The short version is that they're all decisions for your prescriber — but it helps to know what the official guidance actually says. Switching from Wegovy or Ozempic to Mounjaro
Wegovy and Ozempic contain semaglutide; Mounjaro contains tirzepatide. They're different drugs, and there is no agreed dose conversion between them — being on a high semaglutide dose doesn't mean you start Mounjaro at an equivalent 'high' dose.
NHS Specialist Pharmacy Service guidance notes there is very little data on switching between GLP-1 medicines and advises following the product's own instructions for starting doses. In practice that means starting Mounjaro at its standard 2.5mg initiation dose and titrating up, with your prescriber deciding the timing of the switch. Can I increase my dose early?
The dose should only be increased after a minimum of 4 weeks at the current dose. Going up faster doesn't speed up results and tends to bring on more side effects, since each increase is the point where nausea and other digestive effects are most likely to flare. There's no prize for reaching 15mg quickly.
Do I have to keep going up?
No. The maintenance doses are 5, 10 and 15mg, but there's no obligation to climb to the top — the aim is the lowest dose that works well for you. Many people settle and stay on a dose they tolerate. If a step up is hard to handle, options your prescriber might consider include staying longer at the current dose or moving up more gently. What about a weight-loss plateau?
Weight loss naturally slows over time, and plateaus are common. A plateau on its own isn't automatically a reason to increase the dose — that's a clinical judgement. UK guidance reviews progress at around six months, using at least 5% weight loss as a marker of whether treatment is working. Talk to your prescriber about your progress rather than adjusting things yourself.
